So for anonymous, we first get a set of 1 sheet papers, and they have on them (I should mention, this 1 page basic sheet is filled out by the NEDC, based on the bigger sheet):

* 2pn, multi, blasts, & # of them
* donor #
* state of residence
two columns for husband/wife
if one is a donor, husband or wife is crossed out and it's labeled donor oocyte or donor sperm etc.
* age
* race
* height
* weight
* blood type
* eye color
* hair color
* hair type (straight/wave etc)
* skin tone (fair/medium/dark)
* body build (small/medium/large)
* college graduate? field of work (like environmental engineer, stay at home mom)
* ethnic background


We're told to pick a main couple, and enough back-ups to get us 6 embryos (roughly). But there's a couple rules with that. If there's more than 6 embryos, you don't need a back-up. Like stage must = like stage. So 2pn = 2pn, blast = blast. Only exception, is a 2pn is allowed a multi-cell back-up. I'm guessing they may culture the 2pn's to multis, but I need to ask that yet. But no other exception to the match is allowed.

You then can call with a couple you want to look at a little closer. Then you get the full sheets. I should say, I think we had 13 couples to look through in December. And we asked for info originally about 3 of them.

The big sheet is what the embryo donors fill out. It's sent with the same first small short page in front, so we can see who they were again (which donor #, etc). :)
Has first husband, then wife, with this info:
physical characteristics
height
weight
race (circle all that apply) and gives an "other" blank to fill in too
family ethnicity (circle all that apply) (also gives the "other" with a blank)
drug allergies
are you a twin? Identical, fraternal?
hands: right handed, left handed, ambidextrous
Hair:

body (that was all for hair)
Eye color
complexion
body build

bleeding tendency or problem

So then you go over that info and verify that you're still ok :D

Then you call back and say, I'd like donor # ####
and if you need a back-up to get the 6 embryos minimum, then you say, and I'd like donor # #### for the back-up.






On rare occasions, there are anonymous donors who have final say. They view our profile & pictures. That happened to us, and why we were not matched with our first choice in December.




And for open:
The open placers for adoption first view the profiles of the adoptive couples, along with their pictures. It's a brief page and a half about us (something like font 10, no returns, solid page and a half.). Then hmm I think 3 pictures. And then proceed as mentioned in previous posting about how open works.
